About Federal Trade Commission (FTC)

Federal Trade Commission (FTC) was created in 1914 to prevent unfair competition and has taken part in the administration of consumer protection laws in 1938 such as the Pay-Per-Call Rule and the Equal Credit Opportunity Act. How can you request your Credit Report by mail?

Before you begin, you will need to prepare the following:

  1. A computer with internet access.

  1. Access to Free Credit Reports at annualcreditreport.com and provide your name, address, Social Security number, and date of birth and verify your identity to order your free annual credit report by calling 1-877-322-8228, or by completing the Annual Credit Report Request Form.
  2. Mail the Annual Credit Report Request Form to: Annual Credit Report Request Service, P.O. Box 105281, Atlanta, GA 30348-5281.
